Allow application-defined wizard pages in the 
org.eclipse.hyades.ui.reportExtensions extension point.

For example, when an application implements this extension point to create a 
report, the application's report may require more data from the user to 
generate the report (e.g. report window).  The 
org.eclipse.hyades.ui.reportExtensions extension point should be extensible 
enough that applications can provide additional wizard pages to appear in the 
report wizard (e.g. activated from the wizard's Next button).

A new extension point named reportGenerators replace the existing 
reportExtensions.
The contributors to reportExtensions still have their report displayed as a 
choice.

Verified in Toronto's TPTP 4.0 (200503211143 build of HEAD) driver on March 21, 
2005.
