A single unique dialog instead of two dependent dialog to choose the Element to extend

Model Fragment is a bit confusing when choosing Extended Element ID and Feature Name from different dialogs [1]. 

According Bug 467262#c11 the current solution is a bit confusing because two independent dialogs are "magically connected"

- The dialogs "Find Parent Elements" and "Feature Name" are connected in a way that is not visible from the user, depending on user input.
- this proposal is for replacing the two existing dialogs with a new dialog, grouping the features of "Find Parent Elements" dialog and "Find Feature Name" dialog.

I like the proposal but I think even better would be if we would show a "real" model with all possible entries and let the user select from there. The selection via  instead of offering the underlying EMF model, this should be more for an export model.

The dialog could load or create an example model with all entries and let the user select to which we want to contribute. The advantage would be that the user is already familiar with the tree representation of an existing model.

This bug hasn't had any activity in quite some time. Maybe the problem got resolved, was a duplicate of something else, or became less pressing for some reason - or maybe it's still relevant but just hasn't been looked at yet.

If you have further information on the current state of the bug, please add it. The information can be, for example, that the problem still occurs, that you still want the feature, that more information is needed, or that the bug is (for whatever reason) no longer relevant.
