build I20031021

- open Keys pref page
- select command File / Close
- no bindings appear
- Ctrl+F4 and other bindings appear to work OK even though they are not shown
- I had previously defined Ctrl+W for File/Close and it is not working, however 
I had also bound Ctrl+Shift+W for File/Close All and it is working.  This may 
be due to the scope problem for Ctrl+W - I normally have to define it in the 
text scope as well as the global scope in order to work around a (known) bug.  
Close All doesn't have this problem.

Confirmed that Ctrl-F4 appears in the list in build I20031119.

However, it is still necessary to bind Ctrl+W to both the general scope and the
text editing scope. Should we re-open, or is there another bug for this?
