4.3 M7/RC0.

We should make the 'Deprecated API removals' more public in the Migration Guide: - should not be at the end of the TOC
- 4.2 -> 4.3 migration guide should link to it

Also:
 - need to review future removal list to see what was actually removed
 - need a general entry on Lucene migration (see M1 N&N)

This is turning into a general bug on migration guide updates..

 - Add recommended changes item on migrating 2.0-style plugins as shown in the log. See bug 407525 for details on log message.

I have done the following:

 - Moved API removal section to the top of the list
 - Added a link within the 4.3 migration guide to the removals guide
 - Added an entry on incompatible changes in Lucene 
 - Added an entry recommending all plug-ins have an OSGi manifest
 - Removed Update Manager from upcoming removals section (already removed in 4.2).
