The external tools menu, accessed from the run menu sometime just appears with 
the external tools icon and no text. As well there is no cascading menu when 
this happens. Not sure what causes it, but it happens often.

I can reproduce by first realizing the external tools drop down in the toolbar 
and then the Run menu.

Currently the Menu bar menu and the toolbar menu are the same menu as specified 
by the XML.
Until I have more time to look into this I have separated the two and now 
things work.

I'm going to call this fixed now. I believe this is the proper way to do this - 
these are two different actions with the same contents and underlying class.
