When the user clicks Add, a new line is created, but typically the cursor is put into the first field, to make it clear that the line was added and so that the user can begin typing immediately.

The cursor is there, but the user cannot type. Users would either: 1. Expect to be able to type and have the menu jump to the match the menu items with the corresponding letters (if it is read only) or 2. Have the first item in the list selected and the drop down expanded, or a sensible default selected. In this case, there isn't really a good default, I would go with the first item.
Well, there were numerous technical and conceptual problems with having the cursor in the first field, so I put it in the 2nd one. Since the first one is optional anyway, having it in the 2nd one still proves useful. I will probably open up an enh request soon to completely remove the first column as it introduces more problems than it solves.
