Build 20020514.

Looking into the Edit menu when having one default text editor open, you see a 
set of action 3 times (e.g. Go to Line is a member of this set). When closing 
the editor, the set is still 2 times in the menu. Reopening the editor adds 
another appearance. 

This happens for actions and not for submenus.

We have seen this state but have never been able to recreate it.
The fact that toolbar contributions are not duplicated suggest it is a problem 
related to the menu manager rather than a failure to remove action bars.

Which 3 actions?  Is there anything special or different about these actions?
Are they the only menu items contributed by the text editor?

Sorry, I don't have a precise reproducible testcase.  FWIW, I captured the 
screen.  For me, closing all the editors did not remove any of the items.  
Newly opened windows don't have the same problem.
