[ptp-user] [Bug] External tool icon does not appear on the toolbar

Hi. The external tool icon, although activated in the perspective, does not appear on the toolbar (while it is present and works on the menu Run). 

It works on standard Luna release, and if you use the PTP distribution (or if you install PTP on top a standard distribution) the icon is not there.  

Checked on MacOSX+Java7 and Linux+Java8
