I do not think refresh it is useful for debugging. For now I would leave it in ExternalTools.

I'm in the process of moving the launch configuration variable support from 
ExternalTools UI into Debug UI.

Should I move the support for the Refresh tab into Debug UI? Or is it only 
really useful for ExternalTools/Ant?
