Don,
A couple of
things...I tried to re-take the survey, but it "knows" I
took it before and supplies all my previous answers, so I couldn't
re-test the flow. Can you "clear" me out?
Also, here's
some possible rewording on the invitation. Historically the invitation
is a major factor in determining how many responses you'll get.
Starting with a "hook" about the impact of their input
traditionally raises the number of responses.

Plug-in Central portal! The following survey should only take about
five minutes to complete.
On or after
September 20th, 2006 we will hold a random draw from all completed
surveys. One lucky winner will receive the grand prize, a 30 gig iPod!
Four Runner-up prizes of $100 Amazon.com gift certificates will also
be awarded.
