The form I used was:
https://accounts.eclipse.org/user/password?destination=user/edit 

I have received an e-mail an clicked on the link there and set a new password. Afterwards I could not login.

Info: I didn't use the account for a couple of months before.

It looks like the email for this account was reset to it's previous value after the outage a few weeks ago.  I've updated it so you should now be able to recover it.
