ECA text occurs twice on the page

My ECA page shows the text of the ECA twice. I've attached a screen shot that shows the bottom of the first iteration and the top of the second. The second occurrence appears to be plaintext.

Could this be related to Bug 534413?

I think it's related to the removal of:

The last line in the text is out of place without the actual live form.

"You represent that the information provided below is accurate."

There's no "information provided below", so it just looks weird.


Can we revert the change from 534413? 

We will need to re-create the patch and make sure the change does not break the ECA form on account.

I merged a patch to add the line back to the document.
