Update JDT T&T document with relevant items from Mars (4.5) N&N documents.

Added the following N&N items to T&T document:
- Migrate lambda expressions to method references and back
- Shortcut for Skip All Breakpoints
- Annotate command

Updated the list of Quick assists with:
- Convert to lambda expression
- Convert to method reference
- Add inferred lambda parameter types
- Add parentheses around lambda parameter
- Remove parentheses around lambda parameter
