Type pagedown from anywhere in the editor, or type arrow down when on the last line of a page, and the screen reader begins reading the wrong line.

Actually, I can make this happen just with arrow down keys on the first page.

This may (?) be partly explained by "slow caret movement" in the Orion TextView, as described in this Firefox bug comment:
