The label tags associated to the various input form controls currently have a "for" attribute value of "undefined". This causes a screen reader to fallback to guessing what the label for each form control is. In particular, JAWS seems to think that each of the password fields is associated with the word "Password" and not "Old Password" or "Retype password".
