The refactoring context menu should be made extensible. Perhaps it could be modeled after the JDT refactoring menu or something similar.

The menu should also be redesigned to perhaps use the new command/handler mechanism.

Using the new command/handler framework for the refactor action now. The refactor action also works in the source view too.
