When reopeing a diagram (already open) I would expect that it be refreshed. 
Currently, this is not the case and a new tab is created. We can then have several tabs concerning the same diagram, which becomes unusable after some time.

you can also contact Cedric in order he may help you solving this bug that may relates to the backbone of Papyrus.

Some support from the backbone infrastructure is needed to prevent a diagram to reopen in the Sash editor.

This is an expected behavior. So there is no bug :-).
But maybe the action name is not appropriate, so I have change it in "open in new tab".
There is normally no need for a "refresh" action as all diagrams are normally automatically synchronised (thanks to the MVC pattern). Check it : open the same diagram twice and modify one ...
