What happens:

When using the "cleanup document" tool, the OK button is disabled if I try to make it _only_ "insert required attributes". Whenever at least one other cleanup option is enabled, one can click OK and apply the cleanup operations.

What I expect:

You should be able to apply the "insert required attributes" operation by itself.

Tag name case and Attribute name case must both also be set to "As-is".

This was actually released back in M5.
