1) Add context Help to the TOC Editor, including a little blurb in the context description.

2) Add a Help Icon to the upper-right corner of the TOC Definition page that takes you to a corresponding HTML page.

3) Insert linked page into a TOC Editor entry in the PDE Guide TOC using, oddly enough, the TOC Editor. :)

Patch for bug 198686.

- Added Help pages for TOC editor, TOC definition page and new Help TOC wizard
- Changed general "New File Wizards" and "Editors" pages to add relevant links
- Added context help to New Help TOC Wizard and TOC Definition pages
- Overrode original source page context help to display TOC Editor context help
- Added context help blurbs and links

Nice patch Noam, looks great!  We are going to have to upgrade the help/content assist on the other editors to make it as nice as the TOC editor :)

Mike, since it is a holiday in CA I went ahead and committed the patch.  If you have a problem or see something I missed, please reopen the bug and we can address it.
