Wait for the email after the committer vote was approved by PMC. It will contain all information you need. The good things take a while ;)

A propos paperwork:

Chen, as soon as the committer election passed you have to provide some data and fill out a individual committer agreement and the individual committer questionnaire. In order to speed up the committer election process, please read [1], section T in particular, and prepare the individual committer agreement so that we can send them to legal team as soon as the election period ends.
