Two possible problems come to mind:

a) Your plug-in references some other plug-in(s) which are available
in your Eclipse installation but are missing in your RCP application.

b) You used an extension point for your menu contribution that is not
available in your RCP application. This is actually a variation of a).

If your application provides the complete "Help" menu you can have a
look at the "About Eclipse" entry and look into the configuration or
the plug-in details. Also the .log file in the .metadata directory of
your workspace may contain hints on what went wrong.
