One advantage of using [patch] in the summary is that you can use the flag as a dynamic workflow state. For Mylyn we remove the [patch] tag once a patch has been merged or it has been determined that a patch needs to be revised for instance. If you search for patch attachments you will always get all bugs with attached patches regardless of state.

We use [patch] in the bug summary because it's easy to filter for patches in the Mylyn Task List that way and the whiteboard is nested in a section in the task editor requiring more clicks to edit.
