[rt-pmc] New Asterisk account for PMC calls

As discussed on the PMC calls we will move to the new Asterisk account for the next PMC call.  Here is the inforation:

Participant conference extension: 704, then enter pin 12459

* SIP clients can call 704@xxxxxxxxxxxxxxxxxxxx, then enter pin 12459.

I also updated the RT meeting wiki (http://wiki.eclipse.org/RT/meetings) and google calendar linked at the bottom of the RT wiki (http://wiki.eclipse.org/RT).
