I had this issue also some time ago and I clearly remember suspecting the origin is the same as that of bug https://bugs.eclipse.org/bugs/show_bug.cgi?id=391430 . Since I have to maintain only pure Eclipse 4 RCPs I fixed it internally in a way which is incompatible with the compat layer.

Steven, Lars, did you test your fix with the Eclipse IDE (not just pure RCP 4 apps)? I know that these menu issues behave differently if you put the compat layer on the 4.x stuff. Anyway I'll also set aside some testing time today to test this and the latest e4 tooling.

Steven provided a patch for it which works. But before putting this into the maintenance release I would like that someone else validates if that is the correct solution.
