This is a bad limitation, but I do not have membership on another survey tool.

Hi Don,
I just went through the new version and it looks great! I think leaving question 14 as it is will be the best approach given our choices. Its not uncommon for the "Don't Know" choice to be where it is.

I think we're ready to launch next week.

I have incorporated feedback:
 
 - New intro
 - Able to redo survey for testing (sorry Mike)
 - Added Question numbering
 - Noted progress periodically in page title
 - Removed some of the "answer required" flags to help people who decide to rush it
 
Maher raised a really good issue with question 14 that I cannot find a fix for - the "Don't Know" column is on the right, and Survey Monkey does not appear to have a way to put it on the left.  Our options are:
 - Reverse the colums to that High Value is on the left, and No value is on the right (and then Don't know would be far right as it is)
 - Remove the don't know colum (I don't like this idea)
 - Leave as is, and unfortunately miss out on some people who might accidentally click high value.
 
I've opened a bug with Survey Monkey to see if they have ideas, but for now, your preference would be helpful.
