Wayne, I've stripped the generated html down to the bare bones.
Basically updated the template, and the final result is in the
proposal.html file. I corrected spelling mistakes and did some
general clean up.
Attached are the latest files.

the title in the html (this will be done by the php wrapper), I
don't
need to see a table of contents, your name and email address are
not
required, and I would rather the revision history be placed at the
bottom.
The final document will be renamed proposal.html if that matters.
As part of the creation process, we'll have to do a trademark
search/assignment on the name. Do you anticipate any trademark
issues?

to tweak the generated HTML at all.
