[modeling-pmc] [CQ 10225] MDHT initial contribution for CDA

We request Parallel IP Process for this initial contribution. All source was
developed by our team members over the past 5+ years and has previously passed
a thorough IP review process by IBM for release at Open Health Tools.
We have two active projects using and developing this code base, which is why
we have delayed submitting a snapshot for initial contribution. We would like
to get this code into Eclipse git repo, refactor to org.eclipse.mdht namespace,
and continue work while the IP review is underway, We understand the risks
associated with Parallel IP Process.
