[equinox-dev] code reviews and 3.2 shutdown

As we are entering the final stages
of 3.2 please take extra care in releasing code.  Note that we are
in a bug fixing mode and should not be adding any new features (unless
authorized by the PMC).  As a team we should all be reviewing each
others changes and actively seeking out reviews for our own changes.  Attaching
patches for review is a good way of getting feedback and tracking changes.
