[e4-dev] Nasty bug in toolbar

We were chasing a bug with random toolbar item dispersing and NPEs,

and I think we found it. The cause seem to be that cool bar manager remove

wrong items from toolbar. Details and patch here

Would appreciate if somebody can take a look and apply to 4.2 branch since

we shipping a product from that branch
