Integration with Disassembly editor

With bug 326636 the CDT Disassembly view will also be available as editor. 
This patch adds support for this in the TCF debugger to open the Disassembly
editor for any stack frame which has no source associated.
This depends also on bug 329682 for the basic Disassembly view integration.
