[cdt-dev] Wiki on how to extend DSF-GDB

Hi,

thanks to a nice summary of his work by Andy Jin, I've added a section "How to extend DSF-GDB" to the wiki.
It currently explains how to quickly add/remove/modify existing services by using the services factory and how
to add/modify MI commands by using the command factory.

Please feel free to add your own suggestions/experiences.
