I am not quite an expert but as far as I have understood the code the indexer 
is not used for "open declaration / definition". Instead the translation unit 
will be parsed on the fly and then the name under the cursor is searched for. 

"open declaration / definition" is quite unreliable in my experience. I have 
investigated one case in more detail 
(https://bugs.eclipse.org/bugs/show_bug.cgi?id=109506) but do not see myself 
in a position where I can fix anything.

Though I would be quite happy to see this important feature working better for 
me than it is now.
