Another screenshot of the Product Editor.
Seems to be the same as bug 569743. (OS X, that one.) Evidently nobody tested 2020-12 theming on Linux/OS X with forms. :-( It's the same problem with Theming turned off in Preferences. Created attachment 285059 [details]
