The following problem description comes from a tvt tester in Toronto...
While in the manifest editor, it appears she found a reference to the Workbench 
menu in the Add extension wizard.  Please refer to her comments below.

Re-create procedure : 

From the main Workbench panel, open or switch to the Resource perspective. 
In the Navigator view, expand a plugin project and select plugin.xml. 
Double click the plugin.xml resource to open the manifest editor. 
On the lower tabs, click on Extensions, then click on the Add... button.
Select Extension Templates in the left pane and Preference Page in the right 
pane.

In the lower pane, the last sentence is "To see the result, select Workbench 
then Preferences... from the menu bar." However, it is unable to find Workbench 
in the menu bar. Maybe it should be Window instead.
